Young Actors' Theatre Participation Liability and Waiver form. This form is intended to be completed by all participants of Young Actors' Theatre including all cast, crew, volunteers, campers and class attendees. This form MUST be completed before attending any of our programs. If you have not completed this form since August 1, 2026, we will need you to have this completed prior to the first class.
